How do I prepare for my Initial Psychiatric Evaluation?
After completing the new patient paperwork, you will meet one of our board-certified psychiatric providers at our outpatient office
in the community of Cordova in Memphis, TN. During the appointment, your provider will gather information about your personal history,
including family background, medical history, and any relevant social factors.
Coming prepared for your Initial Psychiatric Evaluation will help our team better develop a personalized treatment plan
that addresses your specific needs and circumstances. We recommend you maintain a journal to track symptoms, moods, and thoughts, 
taking special note if any specific events trigger certain behaviors or thoughts.
We also recommend that you bring a list of psychiatric medications taken in the past, being sure to document
how they made you feel and if you experienced any side effects. We also ask that patients provide a list of all psychiatric providers seen in the past and a list
any in-patient and/or out-patient treatment facilities you have received treatment from in the past.

What happens after my initial psychiatric evaluation?
